Prescriptions may be transmitted to an appropriately licensed pharmacy for fulfillment.
The pharmacy is responsible for dispensing, packaging, and shipping medications in accordance with applicable requirements.

Forge Medical Texas, PLLC provides medical services associated with the Easy Weight Loss RX program. Charges related to your Treatment Plan may be processed under Forge Medical Texas, PLLC, and “Forge Medical Texas” or a similar descriptor may appear on your credit-card or bank statement.
You are responsible for all authorized charges associated with your purchases.
Because prescription medications are prepared or dispensed specifically for an individual patient, orders generally cannot be cancelled or refunded once your payment has been processed.
Prescription medications cannot be returned or reused after they have been dispensed.
Consultation, medical evaluation, and other professional fees may be nonrefundable once the applicable service has been performed.
Nothing in these Terms limits any refund, replacement, or other remedy required by applicable law.
Unless expressly disclosed and authorized at the time of purchase, Treatment Plans are not automatically renewed.
